Release from Liability

I understand that I must depend on my own health insurance to provide my care if, in the course of my volunteer work, I am injured, become ill and/or require medical treatment. I understand that New York State Worker's Compensation Law does not cover me.
I am aware that volunteering on Project EATS’ farms pose certain risks and dangers including sunburn; heat exhaustion; back or knee injury due to improper prolonged squatting, lifting, or pulling; personal injury or injury to others caused by a fall or an accident due to misuse of farm tools. After full disclosure by Project EATS, I am voluntarily participating in these activities with the knowledge of the danger involved and hereby agree to accept any and all risks of injury.
By providing my email address, I consent to receiving email from Project EATS. This release is made to allow Project EATS to include me as a subject in connection with materials used by Project EATS for purposes including, but not limited to, publicity, fundraising, marketing, advertising, promotion and the like (hereinafter "Materials"). In connection with this release, I agree that Project EATS and its agents, representatives and employees may tape and photograph me and record my voice and conversation, including quotes, paraphrases and sounds. I further agree that Project EATS shall be the exclusive owner of all copyright and other rights in and to the Materials and that Project EATS may use and license to others my name, voice, likeness and any biographical facts that may have been provided to Project EATS, in connection with the Materials forever and throughout the world, and to license others to use such, in any manner Project EATS wishes in any and all media now known or hereafter discovered or developed. I understand and agree that Project EATS has no obligation to include me in the Materials. I also understand and agree that there shall be no payment of any kind due in connection with this release and the subject matter contained
herein. I have carefully read this agreement and fully understand its contents. I am aware that this is a release of liability and a contract between myself and Project EATS and sign of my own free will.

STANDARD OF CONDUCT
Project EATS is committed to fostering a professional environment. Toward that end, volunteers are expected to conduct themselves in a professional manner: exhibit work characteristics that exemplify courtesy, integrity and good judgment. The objective is to ensure the highest standard of health and safety for all volunteers, employees and
the general public and to provide for the efficient and effective operation of the organization. No volunteer shall be allowed to harass any other volunteer or employee or member of the general public by exhibiting behavior including, but not limited to, the following: verbal harassment; physical harassment; visual harassment; sexual harassment; or carrying items on the organization’s property intended for the purpose of injuring or intimidating, or threatening injury or intimidation. In addition, the unlawful distribution, dispensing, possession, soliciting and/or sale of a controlled substance or alcohol is prohibited at Project EATS. Volunteers are prohibited from appearing on the Project EATS premises or engaging in work while intoxicated or impaired by the use of alcohol or controlled substances.

VOLUNTEER AGREEMENT
We appreciate the time that our volunteers contribute to the organization. Without volunteers, we would be unable to fulfill our mission.
I understand that as a volunteer of Project EATS, I will not receive any monetary compensation or any other form of remuneration from Project EATS. I understand that I am required to abide by the above stated standard and that any behavior inconsistent with these policies may render me subject to warning or action up to and including dismissal.
